1. Acknowledgment of Risks. I understand and acknowledge that the classes involve a risk of injury. The risks include, but are not limited to, slips and falls, collisions with other participants, misuse of equipment, improper landing, and pre-existing medical conditions becoming aggravated. I freely accept and expressly assume all risks arising from my child's participation in the classes.*

2. Waiver and Release. By allowing my child to participate in the classes, I, on behalf of myself, my child, and our heirs, executors, administrators, legal representatives, and assigns, agree to waive, release, and not bring any claims or lawsuits against any instructor, facility owner or operator for any injuries, damages, losses, costs, or expenses (including attorneys' fees) that my child may experience while participating in the classes, except for willful misconduct or gross negligence.*

3. Pre-Participation Disclosures. I will inform instructors of any medical conditions, limitations, allergies, medications, or special needs that may affect my child's safe participation.*

4. Photo and Video Release. Unless l indicate otherwise in writing, I grant permission for photographs, video, and audio recordings of the Participant taken during the Activities to be used by the Released Parties for promotional, educational, or archival purposes without compensation. I understand that such media may be used in brochures, websites, social media, newsletters, and other media. Please send a message or otherwise notify miss Amina if you do not want your child to be photographed. *
